The truth about the harm and devastation the HPV vaccine has and will continue to inflict on children and their families needs to be shared. We are asking you to help us inform the public, and our State and National Representatives by sharing personal stories of HPV vaccine-injury.

Rules:
1. Keep your video under 30 Seconds.
2. Record your video horizontally.
3. Submit a written version with more details of your experience in the field provided on the form. (optional)
4. Submit by January 5th for first edition edit.
5. Submit by January 10th for 2nd edition edit.
6. By submitting your video you agree to release the rights to your video to be used in our campaign and within the Medical Freedom Movement and acknowledge your video may or may not be used to share on social media and any public platform.

Before You Submit:
1.Make sure your written testimonial is factual, does not contain shock culture comments intended to stir up anger or dissent.
2. Check that your video is recorded horizontally.
3. Check that the sound on your video is clear and there is not too much background noise.
4. Make sure to provide your email to have the edited version sent to you as soon as it is available.
